Incident Investigations & Long-Term Solution

This protocol applies when environmental incidents arise due to the lack of, and the implementation of environmental policies and procedures. These incidents are identified, and the immediate environmental impacts/risks are mitigated. The long-term solutions are determined based upon incident investigations, root cause analysis, and corrective actions. 

Documented information required to be retained for incidents includes incident description, risk-assessment of the proposed action, actions taken, and the authority of the person deciding the actions for incident management.
